**Job Title: Home Health Registered Nurse (RN) / Licensed Vocational Nurse (LVN)**

**Position Overview:**
We are seeking compassionate and skilled Home Health Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to join our dedicated team in Webb County, Texas. In this role, you will provide high-quality, patient-centered nursing care in the comfort of patients' homes. You will work collaboratively with healthcare teams to ensure that our patients receive the best possible care tailored to their individual needs.

**Key Responsibilities:**
- Assess patients' health status and create personalized care plans in collaboration with healthcare providers, patients, and their families.
- Administer medications and treatments as prescribed, including intravenous (IV) therapy, wound care, and health monitoring.
- Educate patients and families on managing conditions, medication management, and promoting overall health and wellness.
- Document patient care activities accurately and in a timely manner in accordance with regulations and organizational policies.
- Monitor and record patients' vital signs and health status, reporting any significant changes to the healthcare team.
- Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
- Provide emotional support and reassurance to patients and their families, fostering a positive and compassionate care environment.
- Adhere to infection control protocols and safety guidelines to ensure the well-being of patients and staff.

**Qualifications:**
- Current and valid Texas RN license for Registered Nurse positions or LVN license for Licensed Vocational Nurse positions.
- Previous experience in home health, acute care, or a related nursing field preferred but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Basic knowledge of home health regulations and compliance standards.
- CPR certification required; additional certifications preferred (e.g., ACLS, PALS).

**Physical Requirements:**
- Ability to lift and carry up to 50 pounds.
- Must be able to bend, stoop, and stand for extended periods of time.
- Capable of navigating safely in a home environment including stairs, uneven surfaces, and limited space.
