**Salary:** $55,000-$75,000 per year

**Job Summary:**  

We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to join our team. The ideal candidate will possess clinical expertise and a strong commitment to delivering quality patient care in a home setting. As a Home Health RN, you will play a vital role in assessing, planning, and implementing nursing care for patients in their own homes, ensuring they receive the support they need to maintain their health and independence.

**Key Responsibilities:**

- Conduct comprehensive patient assessments to develop individualized care plans.

- Administer medications and treatments as prescribed by physicians.

- Monitor patient vital signs, progress, and overall health status, providing necessary interventions.

- Collaborate with physicians, caregivers, and other healthcare professionals to coordinate care.

- Educate patients and their families on health conditions, medications, and home care techniques.

- Document all patient interactions and changes in condition accurately and in a timely manner.

- Ensure compliance with all state and federal regulations and maintain patient confidentiality.

- Assist patients with activities of daily living (ADLs) as needed.

- Participate in team meetings and continuing education opportunities to improve care quality.

- Provide emotional support and counseling for patients and families in navigating health challenges.

**Qualifications:**

- Current Registered Nurse (RN) license in Texas.

- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.

- Minimum 2-3 years of nursing experience, with at least 1 year in home health or community health setting preferred.

- Strong clinical assessment and critical thinking skills.

- Excellent communication and interpersonal skills.

- Ability to work independently and manage time effectively.

- Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification preferred.

- Proficient in the use of electronic health records (EHR) and other relevant technology.

- Valid driver’s license and reliable transportation to travel to patients’ homes.

**Physical Demands:**

- Ability to stand, walk, and lift up to 50 pounds as needed.

- Capability to perform tasks requiring physical endurance, including bending, kneeling, and reaching.
